Start with your numbers. We’ll work through the rest.
The standard Medicare Part B premium is a monthly cost; the deductible is a separate annual amount. Our 2027 planning figures are $209.50 per month and $292 for the annual deductible. Both are estimates, not official CMS announcements. The verified 2026 reference figures are $202.90 and $283, respectively. The cards above keep these statuses visible so that a projection is not mistaken for a final bill. Your own payment may also include an income-related adjustment.
What the standard premium tells you
The standard premium is the starting amount for Part B coverage. It does not describe every Medicare cost. You may also have an annual deductible, coinsurance, a drug plan premium, supplemental coverage costs, or an income-related adjustment. Keep those categories separate when you prepare a monthly spending plan, because not all of them are withheld from the same payment.
The 2027 IRMAA brackets explain how income can increase Part B and Part D costs. For 2027, the income thresholds shown on this site are projected and the surcharge amounts remain not yet released. A standard premium estimate is therefore insufficient to calculate a confirmed total for a person who will owe IRMAA.
How much of your COLA goes to Part B
A Social Security COLA raises the gross benefit. A higher Medicare deduction can absorb part of that increase before the payment reaches your bank. The comparison tool starts with the gross COLA dollar increase and subtracts the change in the standard Part B premium. It shows the remaining change under those assumptions, rather than treating the COLA percentage as a guaranteed deposit increase.
Using the data currently available, the estimated monthly standard premium change is $6.60. That difference comes from a projected 2027 amount compared with an official 2026 amount, so the difference is itself an estimate. Enter the correct gross benefit in the Social Security COLA calculator to see how the comparison relates to your benefit.
Read your gross benefit and deductions separately
If Medicare is deducted from your Social Security payment, the bank deposit is a net amount. Using that deposit as the gross benefit understates the starting amount for a COLA estimate. Your benefit notice or online account should help you distinguish the gross benefit from Medicare, tax withholding, and other deductions. Match each figure to its label before entering it.
The mini tool compares standard Part B premiums only. It does not determine whether a hold-harmless provision applies, recalculate IRMAA, or account for every personal deduction. Some beneficiaries pay Medicare directly rather than through a benefit deduction. For them, the premium still affects household cash flow even though the Social Security deposit may not display the change.
The deductible is not a monthly premium
The annual deductible is a cost-sharing threshold, not an amount to subtract from every monthly benefit check. How much you actually pay also depends on covered services and any other coverage you have. The deductible estimate can help with a reserve for medical expenses, but it should not be added to each monthly premium payment as though both recur on the same schedule.
When comparing years, compare monthly premiums with monthly premiums and annual deductibles with annual deductibles. If you annualize a monthly premium for a budget, label that total clearly. The retirement savings calculator can include health costs within the spending amount you choose, but it does not predict medical use or insurance reimbursements.
Check which information has been confirmed
The CMS 2026 premium and deductible fact sheet supports the reference amounts. The 2027 standard premium and deductible here come from a Trustees projection, while the 2027 CMS fact sheet has not been verified in our current data. Visit the sources and update log to inspect those distinctions. Your Medicare and Social Security notices remain the appropriate documents for your personal payment.
Frequently asked questions
What is the 2027 Medicare Part B premium?
The current planning estimate is $209.50 per month for the standard premium. It is not official. A person who owes an income-related adjustment will have an additional amount, and the 2027 surcharge figures are not yet released in our data.
What is the estimated Part B deductible?
The 2027 annual deductible estimate is $292. It is separate from the monthly premium. The official 2026 deductible is $283; do not substitute that reference figure for a confirmed 2027 amount.
Will my Social Security deposit rise by the full COLA?
Not necessarily. The COLA applies to the gross benefit, while Medicare and other deductions affect the net payment. Compare the gross increase with changes in your own deductions rather than applying the COLA directly to a bank deposit.
Does everyone pay the standard premium?
The standard premium is a reference amount. Higher-income beneficiaries can owe IRMAA, and other circumstances can affect what someone pays. The mini tool is not a determination of assistance eligibility, late-enrollment penalties, or hold-harmless protection.
Is the deductible deducted from Social Security each month?
It is not a recurring monthly Part B premium deduction. It is an annual cost-sharing amount associated with covered services. Check the explanation of benefits and your coverage documents to understand the charges for a particular service.
When should I replace an estimate in my budget?
Use an official CMS release to update general planning figures, then compare your own benefit or premium notice. A change to the standard premium alone does not confirm a personal IRMAA charge, drug plan premium, or other deduction.
